Course Description

Transitions of care to skilled nursing facilities can be complex for residents, families, and care teams. Join us as we consider the importance of documentation in the facility setting.

Participants of this webinar will:

  1. Review the critical role of accurate clinical documentation and effective interdisciplinary collaboration in ensuring smooth transitions

  2. Explore documentation strategies that support quality outcomes for both residents and the skilled nursing community

Jackie Nordhoff, RN,BSN, Director of Regulatory Affairs, PointClickCare

Jackie Nordhoff has 30+ years of LTPAC management and consulting experience at the regional and corporate level.  Prior to PointClickCare she was the Vice President of Care Management for Sava Senior Care.  She has extensive knowledge in all aspects of clinical workflows, MDS & RAI, Clinical Reimbursement, Quality Measures, Regulatory Compliance, and Provider operations. Jackie has a passion for education and uses that to help customers realize the regulatory value within PCC.  She sits on multiple state and national regulatory committees.

 
 

Rose Scott-Powell RN, BSN

Rose Scott-Powell has been at PointClickCare for over 10 years and is energized at the opportunity to support those caring for our seniors. Her experience working with hospitals and providers in the SNF and SL space gives her a unique perspective and deep understanding of operations and clinical care.  Throughout her career, she’s embraced change and been a change agent.  As a Market Development Specialist, she  engages with customers, understanding their goals, and assisting them with EHR utilization.

Rose began her career in healthcare as a critical care nurse, moving into the critical care preceptor and manager role. Her career has been focused on healthcare and technology with the goal of improving care outcomes and care coordination.
